I saw one on ClickBank a while back, but frankly, considering how shitty their security is, I'd rethink using it for something that has big commissions attached, like credit or debit cards.
CashCards.net has a whole hokey MLM-esque thing built right in, as well as convertibility from/to several different digital currencies. They've been around for years -- just got one of their C3 cards and it works fine.
