About to try a landing page oppose to direct linking
in ppc but i've got a little predicament that im confused about.

Lets say the offer needs someone to fill out a little tiny form.

How do you implement that on your landing page to submit to the
exact same destination and still get the lead? I dont get it.

exact same destination and still get the lead? I dont get it.
You'll either need to collect leads for your own use, or direct them through your landing page to the advertiser's page.

Unless you have some sort of white label deal going on, you can't collect affiliate leads via your own page.
 
A landing page should catch the attention of your visitor and encourage him to click through the offer.

The user should fill the form only on the advertiser landing page ( unless, as pfgannon said, you have some custom agreement with the network/advertiser)

The main purpose of a landing page is to prepare the user for what he will be required to do do in the next step, moreover you should use your lp to collect data about traffic using something like google analytics, statcounter or what you prefer.
