Pay Per Call/Phone Sale

I've been looking to get into some TV and print offline advertising, and I've been searching for affiliate programs that offer a pay-per-call (with phone number) or pay-per-sale by phone program. Having a web component would be a plus (to market both phone & web), but the phone component is what I'm really after. I'm in CJ's RingRevenue program, but there are very few advertisers in there right now, and none with a product or commission structure that fit my needs. LinkShare also launched a pay-per-call product with RingRevenue, and I'm waiting on approval on that right now.

Any recommendations other than those two, on affiliate programs that have a phone call component where I get my own tracking phone number(s) to use? I don't need a network, and I'm willing to work directly with single-product/service advertisers that offer this kind of service.

moreniche sites have 1-800 #'s on their sites and activation codes (like your AFF ID) which the operators ask for, in which you are rewarded the sale
 
One idea, which I have not got around to doing yet, would be to contact Ring Revenue and try and get a list of all networks and advertisers they're working with. I know Advaliant/MediaTrust was using them too, but they only had 1 campaign going last I checked. Would love to see some more CPA networks adopt the platform.

Unfortunately, I'm trying to stay away from offers that track by using the affiliate ID as a promo code or extension - Just another step in the process that increases breakage, and I'm sure people will forget to write down the code, but remember to call the main phone number. Thanks for the suggestion, though!
 
Looks like Linkshare has a better set of advertisers than CJ in their RingRevenue program - It was a bitch to actually get approved for their network (having had an account with them for nearly 9 years without a single dime of revenue didn't help), and only approved for 2 offers initially, but it looks like the best solution out there so far.

I'll definitely post some basic case study here if I end up trying Google TV ads for one of the offers. It's a new medium for me, but I think being able to run a TV ad with a phone number to call and a website to visit should help to maximize effectiveness.
