I saw it at two networks, but the creatives won't work for me. I need a 468x60 banner. There's lots of different creatives available, but all too big.

Did you ask your AM for it? If they say no then simply leave and never go back. Or make it yourself.
 
you can use any creative you want, I suggest making your own and start split testing. As far as traffic, you can use any type of traffic other then incentivized. Most advertisers creatives suck if you ask me. We NEVER use them personally and always create our own.
 
Try out 424media.com I spoken to someone there and they have green tea, pomegrant offers, as well as other supplemental stuff. No idea what the payout is.
 
The Pomvie on neverblue has been taken from Hydra which pays 34$ by default. For volumes you can ask upto 42$ knowing that they take 25% for themselves by default.

POM Cleanse Diet on CX DIgital pays 28$ by default and 32$ on ClickBooth, i dont like clickbooth though.
