Seeking Quality Landing Pages to Gen good CPC

I'm seeking someone who can design very well landing pages. It will have a Call to Action but more importantly it will have form that needs to be submitted to generate a qualified Lead. I'm hosting the website so the scripts needed for the form to work will be hosted by me.

This website is "DEBT" related so those with past experience with any debt landing pages will be reviewed first. I would like to see examples of work and please dont contact me if your work is boring, dull or flat looking. I need some professional landing pages.

Requirements:

1. Experience and good examples of work
2. Source files to be given to me when completed psd ect.
3. Form validation for the fields Name, Debt amount, Phone #, State

I've been noodling with landingpagerobot.com and landingpageclassroom.com templates. They need a tweak here and there but it's not a problem. PSDS are included with landing page classroom.
