There was a recent documentary in the uk about this as Terry Pratchett was considering what to do regarding his alzheimers and he went to watch a chap who wanted to take the assisted suicide option to preserve his dignity before his incurable disease took it all.

"On July 29, 2011 David hosted a live assisted suicide on Battlecam in which a 62 year old Russian man named Nikolai Ivanisovich with brain cancer was giving a lethal injection at a clinic in Switzerland. The film shows Ivanisovich in bed with a woman sitting beside him, two doctors, & Mr. David holding a laptop with a cameraman filming the event. A doctor talks to Ivanisovich in French about the procedure while the other prepares the syringe with a clear liquid & injects into the IV line. A cardiograph beeps in the background & around 20 seconds after the injection the cardiographs flat-lines. A doctor escorts the woman out of the room while the other checks Ivanisovich pulse with a stethoscope, after a few seconds the cardiograph begins to beep again and Ivanisovich starts to have a seizure at which point the doctor yells for assistance and leave the room. Mr David walks over to the bed & looks at Mr. Ivanisovich, picks up a pillow at the foot of the bed & began to smother Mr. Ivanisovich with it. The film ends with Mr. David on top of Ivanisovich with the pillow over his head as the camera fall & goes to black, at which point it was made clear that this was a hoax.
