Create online "test", and lead capture

I'd like to create a 40-50 question test, capture lead info for the test takers, allow them to see their score, score vs. others etc, and perhaps to make recommendations on how to improve the test/add new Qs, make arguments to change test answers.. I'm looking for a packed solution if anyone knows of one I'd be very thankful..
