For those with Facebook Business Accounts

How did you setup the account?

When I try to setup a business account, it asks for:

"First Name"
"Last Name"
"Date of Birth"

Where specifically did you go to setup a business account, and what information did you provide to establish the billing information?

I believe you have to create a campaign first, then they ask you for the billing info.

Yeah...I created a campaign first. Then it asks for "first name, last name"

How do you specify it's a business account?

We have multiple business accounts, and they are usually given out with an account rep and a mid 4 figure daily budget to start.
