Looking for a form / widget builder.

I need a custom form or widget. That asks for info then plugs that info into a link after they hit enter.
Example: Real Estate
Basically gives them a choice between Buying or Selling a home.
They asks for a zip code.
After info in entered and they hit submit. I want a custom link made out of that info.
