We are looking for a complete solution for lead collection - from a form to put on a landing page to the database the data will be stored in after. The form part should be able to be placed in an iframe on a page so we can constantly be testing the design around the form to see what generates us the most leads without changing the form. The form will be 4 pages long in total with some sort of button on the bottom to move to the next page. It should check for required categories each time before allowing the user to move into the next page and alert them to any spots that haven't been filled out that are required. On the 3rd page at first there will only be a single question asking the user "yes or no" if the user clicks no it will simply move onto the next page, if the user clicks yes a section below will expand to collect a few pieces of information. It's also important that any information put into the form is stored into the database no matter how far the user gets on the form and even if they never click the "next" or "submit" buttons. If the first page asks for "name, email and phone" and the user inputs the info but then doesn't even click submit we still want that info to be stored.
The key thing is the form should just flow nicely - 99% of the forms I see on the web are complete crap and I'm not looking for something like that.
We also are looking for someone to set up the interface to access where the information is put. So at any point someone can log in and see all the leads that have been collected, what time they were collected as well as leave notes about each lead that will be stored in the system.
I know how to figure out code usually a bit from looking at it so the interface can be very simple without the need to be able to create users or anything like that - just have them listed straight in the file for editing or something.
Looking for someone that can do this project quickly and efficently and has some prior examples of work to show you know WTF you are doing.
