kall8.com has been around awhile and is rock solid, basic toll-free is just $2/mo and you can use them for voicemail as well to keep your incoming voice messages separate.

They have a $10 plan and a $15 plan that pretty much can cover anything. Unlimited minutes along with the option to get a 800 number at no additional cost.
