I need someone who is able to design a custom contact form for lead generation based inside a Wordpress page displayed on the front page of the websites.

I'm not talking about something simple like contact form 7, I need something that looks sexy and will stand out.

Have you looked into wufoo? It's hosted so may not exactly be what you may be looking for, but it offers some downright sexy forms and plenty of options. Ports out to mailchimp if you happen to use that platform, as well as a decent selection of other platforms.

You can even get a free account to try it, but you have to upgrade ($30/month) to use the thank you page functionality.
